JST trauma orthopedics rounds are traditional activities of Beijing JST hospital, which are scheduled to be held every Tuesday morning. On August 13, 2019, professor Robert Medoff, a famous American orthopedic doctor, served as the special guest lecturer in the academic activity of ward inspection. He shared with us the "concept of specific fixation of distal radius bone mass", "innovation of elbow fracture", "ankle project" and other topics. Zha Yejun, Associate Chief Physician, and Gao Zhiqiang also shared their views on the treatment of complex fractures of the elbow and distal radius.

Professor Robert Medoff first interpreted the bone block specific fixed concept, in the early 90s, he was thinking of an improved method to improve the unstable distal radius fractures fixation. He classified the distal radius in a specific way, combined with the specific design fixation-TriMed fixation system, which was used to improve the stability of periarticular fractures when using K-wire previously. Since 1994, with the gradual popularization of clinical application, it has been included in “Campbell orthopedic surgery” and “Green hand surgery”, becoming the model of periarticular fracture treatment in the United States.

Intra-articular fractures of the distal radius can be divided into 5 types according to the site: 1. styloid process of radius; 2. Margo volaris/tear drop angle 3. Free bodies joint or die punch 4. Dorsal lateral wall 5. Dorsal ulna angle. TriMed’s fixation system has hook plates, pin plates, supporting accessories, sleds and so on. It can fix the periarticular fractures in terms of various sites to restore joint surface flatness, free the joints and do early functional exercises.

 

He shared with us the clinical imaging data of the treatment of comminuted fractures in the distal radius, olecranon fractures in the ulna and medial and lateral malleolus fractures with the TriMed fixation system.

The next phase was interaction time. Director Zhang Lidan asked: Why is there an arrow mark around the sliding hole of the sidewinder plate? Dr.Robert answered: compression plates are necessary for many fracture surgery. But when lateral malleolus fracture is operated, everybody basically can think of method to prop up the length of fibula. At this time, if you screw too hard, it would be changed into a compression plate. This, in turn, shortens the fibula, so markers are designed as reminders

Associate chief physician, Yang Shengsong: What is the function of the hole under the flank of the sidewinder plates? Dr.Robert: first, it would be easy to bend when doing compression. Second, many patients would ask to remove the fixation plates after fracture healing. The hole can help to remove it by loosing the flank.

During the cases sharing and discussion phase, Gao Zhiqiang, the physician first shared the TriMed-WorkShop practical operation, a cadavers class, which he conducted at the Hawaii academic conference in November 2017 and some imaging data of related cases used of TriMed internal fixation system based on the concept of bone block specific fixation before and after surgery for complicated distal radius fractures collected by him. Postoperative images of the patients all showed good fracture reduction and firm fixation, and early joint function exercise could be carried out without plaster braking after the operation to avoid joint rigidity.

Then Zha Yejun, associate chief physician shared some experience and lesson during the treatment of the ulna olecranon fracture. One of them is elbow comminuted fracture. Olecranon hook plate was adopted for the first procedure. But he found the olecranon fragment was turned and displaced, which cause the fixation failure when doing the follow up one month later. The reason he analyzed were that the fracture line was special, and the tines position was more outer. So the fragment inside was displaced due to the traction of triceps muscle of arm. Therefore, the sled plate was adopted when doing the second procedure. After contouring, the plate fitted the joint and fixed it. The post-procedure recovery was great. Joint activities were normal when following up the patient. Dr.Robert pointed out: the internal fixation part itself is neither good nor bad, but must be used in the correct site. This view was recognized by the scholars present.

In Jishuitan hospital, the clinical cases encountered have been greatly improved in terms of complexity, quantity and postoperative quality. Meanwhile, Professor Robert Medoff's knowledge theory and scientific contributions should also be learned more. At this point, the ward rounds were successfully completed. The attendees felt the new concept of specific fixation of bone blocks and a new trauma orthopedics technology.
